  FILING A CLAIM WITH THE SOCIAL SECURITY SYSTEM (SSS)

    Where to File a Claim

Employees' Compensation claims may be filed at the SSS city/provincial branch where the place of work is located.  

The System through the branch where the claim is filed, shall process the claim upon receipt and determine if the contingency is compensable. If the supporting papers are insufficient, the System may require the submission of additional proofs. The System shall evaluate the claim upon submission of the complete requirements, and the decision either denying or awarding compensation shall be made promptly. Claims Filing

Requirements in filling EC claims thru the SSS

 For Sickness/Accident:

  1. Certificate of employment (in company letterhead, signed by general/personnel manager)
  2. Complete job description (in company letterhead)
  3. Pre-employment medical check-up or in its absence, a certification of the company that worker is physically fit when employed.
  4. Police report (for accidents outside company premises)
  5. Medical findings and/or clinical abstract (ECG tracing & findings, X-ray results & findings, CT Scan, etc.)
         SSS forms to be accomplished:
         * Employer's Notification (SSS Form B300)
         *  Sickness/Accident Report (SSS Form B309)
         *  Sickness Benefit Application for separated members (SSS Form CLD-9A)

 For Disability:

  1. Certificate of employment (in company letterhead, signed by general/personnel manager) 
  2. Complete job description (in company letterhead) of work performed when contingency happened
  3. Pre-employment medical check-up or in its absence, a certification of the company that worker is physically fit when employed.
  4. Medical findings and/or clinical abstract (ECG tracing & findings, X-ray results & findings, CT Scan, others)

           SSS forms to be accomplished:
         * Disability benefit form (BPN 102)
         *  Sickness/Accident Report (SSS Form B309)

 For Death:

  1. Certificate of employment (in company letterhead, signed by    general/personnel manager)
  2. Complete job description (in company letterhead) of work performed when contingency happened
  3.  Pre-employment medical check-up or in its absence, a  certification of the company that worker is physically fit when  employed
  4. Medical findings and/or clinical abstract (ECG tracings & findings,  X-ray result and findings, CT Scan, others)
  5. Police report (if accident occurred outside company premises)
  6. Sickness/Accident report duly signed by employer
  7. Death certificate (certified true copy by civil registrar)
  8. Marriage contract (certified true copy by civil registrar)
  9. Birth certificate of the diseased worker (if single), certified true copy by civil registrar
  10. Birth certificates of children below 21 years old (certified true copy by civil registrar)
  11. Original receipts of funeral expenses

           SSS forms to be accomplished:
         * Death benefits claim (BPN 104) Report of Death (BPN 105)
         *  Funeral Benefit form (B103)
